Located in Hayward, we proudly provide services throughout Alameda County. Our care is designed to meet patients where they are—physically, medically, and emotionally—while working closely with physicians, hospitals, and families to ensure continuity and safety of care.
What We Do
Aloha Home Health Services is made up of licensed clinicians, therapists, and healthcare professionals experienced in delivering in-home medical care. Our team works under physician orders and established plans of care to ensure patients receive appropriate, timely, and coordinated treatment.
We collaborate closely with physicians, hospitals, discharge planners, and families to support recovery and reduce unnecessary hospital readmissions.
What Is Home Health Care?
Home health care is medical care provided in a patient’s home and is typically ordered by a physician. Services are short-term or intermittent and are designed to treat medical conditions, manage symptoms, and help patients regain or maintain functional ability.
Home health services may be covered by Medicare, Medi-Cal, or private insurance when eligibility criteria are met.
